Embeddable Intake Form & Website Widget Manual

Dream Center Connect enables charitable organizations to seamlessly accept community care requests directly from their public websites, church portals, and community partner pages.

Administrators can configure the form, preview it in real time across desktop and mobile form factors, and choose between direct links, responsive iframes, or fully styled standalone HTML templates.

1. Live Interactive Preview

The intake form generator includes a live interactive sandbox allowing administrators to test constituent interactions before deploying to production:

  • Viewport Toggle: Switch between Desktop View and Mobile View to verify responsiveness on handheld devices.
  • Urgency Simulation: Toggle between standard and expedited emergency turnaround requests to observe field styling and notification badges.
  • Category Filter Simulation: Test multi-category selection chips (e.g. Food Support, Housing Assistance, Utility Relief).
  • Contact Channel Selector: Verify preferred notification method inputs (Phone Call, SMS Text, Email).

2. Integration & Deployment Options

Administrators can deploy the intake form using any of three integration methods:

  • Zero Code Required: Pre-binds submissions to your organization or designated triage facility.
  • Mobile Optimized: Full-screen progressive web app experience.

3. Step-by-Step Deployment Guide

  1. Open Embeddable Form Subpage

    Section titled “Open Embeddable Form Subpage”

    Navigate to Org Management > Embeddable Intake Form from the sidebar.

  2. Select Partner Location

    Section titled “Select Partner Location”

    If your organization operates multiple service locations, select the desired Partner Center from the dropdown to route submissions to that facility’s queue.

  3. Preview & Validate

    Section titled “Preview & Validate”

    Toggle between desktop and mobile previews, confirm that required fields and assistance categories display correctly.

  4. Copy Code & Publish

    Section titled “Copy Code & Publish”

    Click Copy Embed Code (for iframes) or Copy Standalone HTML and paste into your CMS page builder. Publish the page to begin receiving community requests.

4. Security & Abuse Prevention

All submissions through embedded forms pass through our enterprise protection gateway:

  • Transport Security: Enforced TLS 1.3 encryption across all communication pathways.
  • Intelligent Rate Limiting: Throttles rapid programmatic submissions to prevent spam.
  • Data Validation & Sanitization: Validates phone numbers against standard E.164 formats, verifies valid ZIP codes, and strips malicious markup before database entry.
